Family Care Overview

Family Care is the name for the redesigned long term care system currently being piloted in nine Wisconsin counties. Specific goals of the Family Care program are to:

  • give people better choices about where they live and what kinds of services and supports they get to meet their needs,
  • improve access to services,
  • improve quality through a focus on health and social outcomes, &
  • create a cost-effective system for the future.

Member-Centered Care

Members of Richland County Family Care are the center of a Care Management Team that determines the services that are necessary for a member’s health needs, goals and preferences.

As part of the Care Management Team, members select those services that are of good quality, meet personal outcomes, and are cost effective.

When health, needs, goals, or preferences change, services change right along with them.

Care Management Teams assist members with the coordination of needed services not offered by the Family Care benefit.
