Family Care Overview
Family Care is the name for the redesigned long term care system currently being piloted in nine Wisconsin counties. Specific goals of the Family Care program are to:
Member-Centered Care Members of Richland County Family Care are the center of a Care Management Team that determines the services that are necessary for a member’s health needs, goals and preferences. As part of the Care Management Team, members select those services that are of good quality, meet personal outcomes, and are cost effective. When health, needs, goals, or preferences change, services change right along with them. Care Management Teams assist members with the coordination of needed services not offered by the Family Care benefit. |
