12 Questions to Ask: 1. What were the results of the most recent state and/or federal inspection and are the results readily available for review? Pine Valley's annual state surveys have been outstanding as evidenced by no substandard care deficiencies. This is achieved by a very small percentage of nursing homes each year. State Survey information on any nursing home is available from the Bureau of Quality Assurance. 2. What type of care does the facility provide? Skilled nursing - 24 hour licensed nurse coverage. Intensive rehabilitation - (2 or more therapies). Alzheimer's care - Special Unit with safety and environmental modifications and staff with specialized training. Personal care - provides assistance with bathing, dressing, feeding and ambulation. Respite Care - provides care from 1 to 14 days. 3. What type of payment sources are accepted, and will the facility directly bill private insurance companies? Medical Assistance (M.A.) Medicare V.A. Private pay Private insurance Pine Valley accepts all of the above payment sources and will directly bill insurance companies. 4. What therapies are available on-site? Physical Occupational Speech 5. How frequently do physicians and nurse practitioners see residents? Ten (10) physicians and two nurse practitioners provide care for our residents. One or more of these healthcare providers are here five days/week. 6. Are rehabilitative therapies and activities available for the residents on weekends? Physical, occupational, and speech therapies are available. 7. What other services are provided? Transportation to medical appointments Podiatry services Dental services Psychiatrist/psychologist services Beauty/barber shop services Religious services Activities Specialty diets Guest trays are available for a nominal fee 8. Are there organized activities designed for all levels of resident participation? Activities are individualized to meet the needs and preferences of residents at all levels of functioning. 9. What is the staffing ratio? Pine Valley exceeds the state staffing ratio requirement for nursing care-givers. 10. Is the Medical Director actively involved in managing the care of residents? Pine Valley's medical director, Robert Smith, M.D., has served as president of the Wisconsin Association of Medical Directors of Long-term Care Facilities. Dr. Smith meets with the administrator and director of nursing on an on-going basis and is actively involved in developing policy and procedures that govern the medical care of our residents. He also schedules routine visits to provide care for our residents. 11. When can friends and relatives visit? Pine Valley has unrestricted visiting hours. Family and friends are always welcome. 12. What is the procedure for handling a resident or family member's concern/complaint? Pine Valley takes any complaint seriously as we are interested in providing the highest quality health care. The social worker or Nurse Manager investigates complaints by conducting staff and resident interviews. After a thorough investigation, the findings and disposition of the complaint or incident are reported back to the resident and/or family member.
