We Offer

Information, Referral, and Assistance:

We provide information and help connect you to the programs and services you need. We can provide information about resources for housing, transportation, home chore/supportive care, health and medical care, residential care/assisted living options, low vision support services, education and employment, food and nutrition programs, legal issues, and public programs like Medical Assistance, Energy Assistance, FoodShare, Social Security, and much more.

Help in Understanding Your Options and Accessing Long-Term Care Programs:

Long-term care involves services and supports that you may need because a disability or chronic illness limits your ability to do everyday things in life. Our Information and Assistance professionals will meet with you and your family to provide objective information to help you evaluate your options and make informed decisions about your current and future needs for care and services. We are the access point for community-based long-term care programs, Family Care and IRIS. We can provide information, help determine eligibility and help you get enrolled into these programs. Family Care and Include, Respect, I Self-direct (IRIS) provide services and supports to eligible adults who are elderly, physically disabled or developmentally disabled that promote independent living.

Transition Services for Youth:

Transition from school to the adult work world takes on special importance for youth with disabilities. We can help young adults aged 17 1/2 and older and their families to successfully transition from school to the adult world. We will provide information about community resources such as housing, independent living resources and disability benefits, and help make connections to needed services.
